7. Who is held responsible if a piece of health information is omitted?

It is solely up to the card holder’s responsibility to declare every necessary information. Failure to do so, omit or lack of awareness is the choice of the card holder.

8. What happens if the server is down?

We have back-up servers that will come into effect.

9. Can I print out my file? Or send it by e-mail? Or by fax?

Yes

10. Who has the right to look at my records?

Anyone who you chose to authorize in writing (signature and date included), or anyone who is present at the time where you are unconscious with the sole purpose to treat you or help you. This is the whole purpose.

11. Can I attach images to my file?

Yes, upon registration you have the option of having the image section open for you, all that for a reasonable extra fee. From then on any X-ray or ECG can be added to your web page or any other image you have and feel would be useful.

12. Can I include insurance or health fund policy numbers in my file?

Yes, we encourage you to do so.
