You probably won't be able to answer every question when you first visit your account.

• So that you can fill out the form at your leisure (and ultimately with any help you may need from your regular doctor), the Vie For Life system allows you to download a PDF version of the questionnaire so that you can return and add to it as often as necessary before finalizing your registration. All you need is your login and password. The card will be created after this step, and you will receive it within two weeks.
